Dual health plans (D-SNP's) include extra benefits to help ease the challenges faced by dual-eligible members.   • Schedule appointments and transportation to your doctor, dentist or pharmacy
  • Review your benefits to help you get the most out of your plan
  • Help make sure you and the people who take care of you at home have what you need

For dual health plan members, a Navigator serves as your single point of contact. Your Navigator is a trusted advisor. Someone who’ll learn about your specific health challenges. Your Navigator will work with you, your family and your caregivers to make sure your care needs are met.
